We accept referrals from general medical practitioner (GP), general dental practitioner (Dentist) and recognised specialist (registered health care providers with Medicare Australia and AHPRA) for any patient who require Oral Medicine services.
Referral letter may be on your business letterhead including patients personal and contact details as well as the reason for referral. For your convenience, please follow the link below to download our patient referral form.
Please send the completed referral form/letter to us directly by fax, email or by mail. Our reception staff will contact the patient to make an appointment at our earliest available appointment schedule.
For Existing Patient
We are in the process of contacting our existing patients. If any one of your patient was seen previously by Dr Firoz Iqbal, they do not need another referral letter for existing condition. Please ask your patient to call us directly on 02 9635 4900 or complete the on-line contact form. We will make an appointment at the earliest available schedule.

Oral Medicine is a specialty within dentistry that focuses on the diagnosis, prevention and predominantly non-surgical (pharmacological) management of a range of oral mucosal diseases, odontogenic (tooth-related) diseases, salivary gland diseases, diseases involving the jaws and associated anatomical structures, oro-facial pain and oral manifestations of systemic diseases including oral health care of medically-complex patients.
Oral Medicine replaces the previously used terminology “Oral Diagnosis” in the current clinical practice.
Oral and Maxillofacial Pathology is a specialty within dentistry that focuses on the tissue-based diagnosis of diseases involving oral and maxillofacial region and relevant anatomical structures using the light microscopy and/or other ancillary technique.
In the current practice, Oral and Maxillofacial Pathology is in use instead of the previously used terminology “Oral Pathology”.
This is a specialist laboratory-based diagnostic and surgical tissue pathology service. Patients are not required to be seen by an Oral and Maxillofacial Pathologist in order to make a diagnosis.
